Important notes
- Mandatory fields: Fill every field marked with an asterisk. Use N.A. if something does not apply.
- Browser: Prefer Chrome, Firefox, or Edge with JavaScript enabled.
- One form per child: Duplicate applications may be rejected.
- Email: Use a valid email so confirmation can reach you.
- Save a copy: Keep a printed or digital copy of your submission.
